Work Readiness Baseline Assessment (DOR Pre-Voc / SDP)

An initial assessment process designed to better understand current readiness levels, strengths, barriers, support needs, communication style, and overall engagement. This helps establish a starting point and identify the areas that may need structure and development before moving into employment-focused planning.

Final Product:
Readiness Report with strengths, barriers, and next steps

Support focused on building consistency, participation, structure, and daily engagement through personalized routines and guided activities. The goal is to help individuals strengthen habits, increase participation, and create a more stable foundation for future growth and employment readiness.

Weekly Routine Plan and Progress Logs

Development of core readiness skills that support greater independence and future workplace success. Areas may include communication, organization, time management, social interaction, responsibility, follow-through, and task completion based on the individual’s needs and goals.

Skill Progress Report

Early-stage exploration designed to introduce individuals to different types of work, industries, interests, and environments in a supportive and low-pressure way. This process helps participants begin identifying what feels interesting, motivating, and realistic for future planning.
